How
to get a replacement Reimbursement Claim Form (SFAA193/205) ? |

The Reimbursement
Claim Form (RCF)
(SFAA193) and the Reimbursement Claim Form for Benchmark Test (SFAA
205) are controlled forms and the forms are sent to all successful
applicants together with Approval in-principle Letter. These forms are
not available for downloading. Applicants who have lost their RCFs
should write to the Office of the Continuing Education Fund or to the
email address cef@sfaa.gov.hk
and request for a replacement copy. Please provide your full name and
the Hong Kong Smart Identity Card Number / CEF application number so
that we can retrieve your application record. We will send the
replacement forms to you by post.

Applications
made on forms downloaded from this website have to be submitted in
paper format in person or by post to the Office of the Continuing
Education Fund (OCEF). Any alternation of the terms and conditions on
the application forms is NOT permitted. OCEF reserves the right to take
legal and/or administrative action against any unauthorized alteration
of the application forms and reject the application concerned.
If
there are any discrepancies between the downloaded forms and the master
copy, the latter one should be treated as the final version. Master
copy of the application forms can be obtained from District Offices and
OCEF.
